This is to inform you about the services you can expect from the Continuum Care, Inc. home health aide assigned to your care.

1.      The home health aide is provided for your personal care. This includes assistance as needed for your bath, grooming, mouth care and linen changes. Other services are provided ONLY as specified on your plan of care.

2.      Our Home Health Aides visit 4 to 6 patients a day. You will be seen between the hours of 8:00 am to 5:00 pm. Although we would like to give you an exact time, we can give you a two hour window. This allows our home health aides to give each patient all the attention they may need on any single day.

3.      Two to three visits per week by a home health aide is considered as reasonable. Adjustments will be made according to your medical needs.

4.      Because of the number of patients seen each day by the aides, their time is valuable. They will assist you to the best of their ability within this time.

5.      Because of schedules, it may be impossible to send the same aide every time, but every effort will be made to do so. Continuity of care is important to us too.

6.      If you wish to have extra hours of care that is in addition to the plan of care, you may elect to pay privately for these services. If so please contact our office to make arrangements on St. Croix at (340) 772-2273 or on ST. Thomas and St. John at (340) 714-2273.

7.      While you are receiving medically ordered services of an aide, a nurse will come every two weeks for supervisory visits. The nurse will evaluate the continuing need for the aide and your satisfaction with the work.

8.      Please note: A home health aide may NOT administer medications. They can assist the patient but they can never dispense medicines.

9.      If you need to reschedule or discontinue your aide visits, please notify the office.
